Understanding De Quervain's Tenosynovitis

Causes

 De Quervain's tenosynovitis is a painful condition affecting the tendons on the thumb side of your wrist. This inflammation can cause significant discomfort, making everyday activities challenging. Named after Swiss surgeon Fritz de Quervain, who first described it in 1895, this condition primarily affects those who perform repetitive hand or wrist movements. 

The exact cause of De Quervain's tenosynovitis is often unclear, but several factors can contribute to its development:

  1. Repetitive Motions: Activities that involve repetitive hand or wrist movements, such as typing, gardening, or lifting, can strain the tendons.
  2. Injury: Direct trauma to the wrist or thumb can trigger inflammation.
  3. Inflammatory Conditions: Diseases like rheumatoid arthritis can increase the risk.
  4. Pregnancy and Postpartum: Hormonal changes and the repetitive lifting of a newborn can lead to the condition.

Symptoms

The hallmark symptom of De Quervain's tenosynovitis is pain at the base of the thumb, which may extend into the lower arm. Other symptoms include:

  • Swelling near the base of the thumb
  • A fluid-filled cyst in the same area
  • Difficulty moving the thumb and wrist, especially when grasping or pinching
  • A "sticking" or "stop-and-go" sensation in the thumb when moving it

Treatment

Treatment options vary based on the severity of the condition:

  1. Rest and Immobilization: Wearing a splint to rest the thumb and wrist can help reduce inflammation.
  2. Medications: Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen can alleviate pain and swelling.
  3. Physical Therapy: Stretching and strengthening exercises can improve tendon function and reduce pain.

Interventional Treatments:

  1. Steroid Injections: Injections can reduce inflammation and pain. They are particularly effective in early stages.
  2. PRP Injection:  Involves using a concentrated dose of the patient's own platelets to promote healing and reduce inflammation in the affected tendons. 


In severe cases, surgery may be necessary to release the tendons and restore normal function.
