Plantar Fasciitis: Causes, Symptoms, and Treatment

Causes

 Plantar fasciitis is a common condition that causes pain in the heel and bottom of the foot. It occurs when the plantar fascia, a thick band of tissue that runs across the bottom of the foot and connects the heel bone to the toes, becomes inflamed. This condition is particularly common among runners, people who are overweight, and those who wear shoes with inadequate support. 

Several factors can contribute to the development of plantar fasciitis, including:

  1. Overuse and Repetitive Strain: Activities that put a lot of stress on the heel and attached tissue, such as long-distance running, ballet, or dance aerobics.
  2. Foot Structure: Flat feet, a high arch, or even an abnormal pattern of walking can affect the way weight is distributed when you’re on your feet, putting added stress on the plantar fascia.
  3. Age: Plantar fasciitis is most common between the ages of 40 and 60.
  4. Occupational Hazards: Jobs that keep you on your feet for long hours, such as teaching or factory work.
  5. Improper Footwear: Wearing shoes with poor arch support or soft soles can contribute to the problem.

Symptoms

 The main symptom of plantar fasciitis is pain at the bottom of the heel, though some people experience pain at the bottom mid-foot area. The pain is usually worse in the morning when you take your first steps, or after periods of rest. It might also flare up after prolonged activity. The pain can range from a dull ache to a sharp, stabbing sensation. 

Treatment

 Treatment for plantar fasciitis typically involves a combination of self-care measures and medical treatments:

  1. Rest and Activity Modification: Reducing or modifying activities that aggravate the condition is crucial. This might mean taking a break from high-impact sports or choosing low-impact exercises like swimming or cycling.
  2. Ice: Applying ice to the affected area for 15-20 minutes several times a day can help reduce inflammation and pain.
  3. Physical Therapy and Strengthening Exercises: A physical therapist can teach you exercises to strengthen your lower leg muscles and stabilize your heel and ankle and calf muscles. Strengthening exercises for the lower leg muscles can also help stabilize your ankle and heel. Wearing a splint that stretches your calf and the arch of your foot while you sleep can help ease the pain
  4. Footwear and Orthotics: Wearing shoes with good arch support and cushioning can relieve pain. Custom orthotic devices can also provide extra support. 
  5. Medications: Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or naproxen can help reduce inflammation and relieve pain.

Interventional Treatments: 

  1. Steroid injection involves injecting corticosteroids directly into the plantar fascia to reduce inflammation and alleviate pain, providing temporary relief for this common foot condition. 
  2. PRP (Platelet-Rich Plasma) injection  involves injecting concentrated platelets from the patient's own blood into the affected area to promote healing and reduce inflammation. 


When other treatments fail, surgery may be considered. This is usually a last resort and involves detaching the plantar fascia from the heel bone.
